This Website i.e. www.shahprachi.com (“this site”) is owned, controlled and operated by Ms. Prachi Nitinkumar Shah Proprietor of M/s. Health Habitat and on her behalf (hereinafter referred to as “Owner”, “Proprietor”, “We” or “Health Habitat”). We request you to refer the About Us page to know more about us while being nutritionists, we offer Programmes titled as Health Pro, Crunch In 90 and Gutreset (details whereof are available on “Our Plans” Page) (Hereinafter referred to as “Services”). We understand and appreciate your concern towards having knowledge about the data, information and contents being shared by you and or made available to us. This page informs you of the policies regarding the collect, use, share and process your information, that you provide to us through your use of the Website as also in the course of availing services that are made available on the Website as defined in the Terms and Conditions to you. Accordingly, PLEASE READ THIS PRIVACY AND COOKIES POLICY CAREFULLY. Also note that only because you have agreed to be bound by the terms described herein, you are using and accessing our Website. In the event, you are having disagreement with the terms of the present Privacy and Cookies Policy herein or any objection in that, kindly immediately stop the further use of the Website.       4. Any additional information that you provide to us during the use / browsing of Website / provided to us while availing of the Services / additional Services or in the course of matters incidental to Services;
      5. Financial information such as bank account or other payment instrument details depending upon the method / mode of payment being availed by and billing information other than payment account number, subject to applicable laws;
      6. Your browsing history including the URL of the site that you visited prior to visiting the Website as well as the Internet Protocol (IP) address of your computer (or the proxy server you used to access the World Wide Web), your computer operating system and type of web browser you are using, the name of your ISP;

  2. RETENTION
    1. The Personal Information and Data as is provided to us is and shall be kept and maintained in our records until the time you continue to avail Services from us. As a part of the availing Services, you would be consulting / visiting us from time to time and in the event we notice that you have not consulted / visited on the time / appointment provided by you and have not followed up for missed time / appointment within stipulated time of Plan, then it shall be construed that you have stopped availing Services from us and accordingly, we shall delete / destroy the Personal Information and Data as is available with us.
    2. In the event of the Services being completed i..e course of programme selected by you being completed, we have no obligation to store and hold the Personal Information and Data and we shall delete / destroy the Data as is available with us.
    3. We may however at our option with no obligations towards you continue to store and hold the information for any research and statistical purposes for a longer period or in the event we suspect or intend to prevent a fraud / future abuse.
    4. The aforesaid storage and holding of Data are exclusively from our perspective of providing effective treatment however we hold no obligations towards you at any time for the storage of Personal Information and Data and consequentially, we are not liable in any manner including and not limited financially.
  3. DISCLOSURE OF DATA
    1. We at Health Habitat make continuous endeavours to make our Services more effective and efficient and for the said purposes, we undertake reviews, discussions, deliberations and research on our own or through persons / agencies as are hired by us. The reviews, discussions, deliberations and research are based on the Data and Personal Information which are available to us to identify the issues and elements and improvise and research upon them. You hereby consent to use of the Personal Information and Data for the purposes of reviews, discussions, deliberations and research and confirm and agree that such Personal Information and Data shall be shared with all persons connected directly or incidentally in the reviews, discussions, deliberations and research. These persons / agencies for and in respect of whom you hereby give consent entities may be located outside India.

      1. Our Service Providers who provide services which are directly, incidentally and or ancillary connected with Services provided by us or are in the nature of the Services required and or essential to us for us to provide Services to you.

    1. We at Health Habitat value your privacy and hence have adopted the advised standards of security for maintaining and protection of Data and Personal Information. We are and shall use reasonable technical measures to safeguard all the Data and Personal Information as is shared with us.

Notwithstanding the above, you are conscious of the fact of the potential issues, bugs, hacking and other issues on account of which, it may resulted into Data and Personal Information being lost, unauthorizedly accessed, safety issue, or any harm caused to you by any misuse of your Data and Personal Information. You hereby acknowledge that we are not responsible, in particular, for any third-party action or action on your part leading to loss, damage, or harm to you or any other person.
    3. For any data loss or theft due to unauthorized access to your electronic devices through which you avail our Services, the Company shall not be held liable for any loss whatsoever incurred by you. Further, you are liable to indemnify the Company as per the Terms of Use.

Upon request being received, good faith efforts shall be made by us to make requested changes in our database. There may be circumstances where we will not correct, delete or update your personal information which shall be decided by us and acted upon accordingly.

    1. We my at anytime amend / change our Privacy Policy and publish the same on our Website. Upon being published, the Privacy Policy shall become effective and binding and notwithstanding of any Data or Personal Information being shared at any point of time, the Privacy Policy upon being amended shall also become effectively applicable to the Data and Personal Information as is shared with us.
